The AWGPN Practice Health Atlas
June 14th, 2007The AWGPN Practice Health Atlas (PHA) is a Decision Support and Practice planning tool for General Practice which is produced by Network or Division staff on an annual basis.
- Epidemiological view of practice data within a geographic information context and informed by practice data, national census data and other data sources
- Clinical and Business Systems Modelling to link the health characteristics of a practice patient population with business opportunities to provide improved health outcomes
- Improved Access to Services for your patients through the use of integrated Human Services Directories
The PHA also utilises the Active Learning PDSA (Plan Do Study Act) approach that is core to the National Primary Care Collaboratives strategy.
The Network or Division can also provide benchmarking data for peer comparisons and with consent use the de-identified aggregated data for regional health planning.
The below is a link to a sample Practice Health Atlas to demonstrate several sections generated for a General Practice; Epidemiology & Mapping and Business Modelling components. The full PHA is approximately 90-100 pages in length depending on the information each Network or Division includes.
